Reporting on support: report card comments?

I'm interested in how you report on support students have received.
We include a comment in narrative reports when students have received tier 3 support, which outlines the support and progress made. This is largely a holdover from when we had a different model. If students have received substantial tier 2 support during the year they'll also get a comment, but it's embedded in the homeroom teacher's comment.
We have recently added OT support and are deciding whether to include that support on reports. For now, we're reporting it separately to parents only.
I'm curious what other schools do: report card comments? SLP/ILP progress reporting only? A combination?

For students receiving Tier 2 or Tier 3 services, this is documented on their report card for that key learning area (e.g. Math). For students receiving Tier 3 services, there is an additional IEP with specific information on the goals that have been set. We are moving towards including therapist goals into the IEP, as these are currently housed in separate documents. (This is largely because our current therapist model is only at 10 hours a week - limited!)
We have been working on integrating the reports though, and this year, have included information on what year level targets are (e.g. for Math) and then documented what the student's individual goals in math may be for that unit, e.g. multiplication. This is largely to document what they are able to do, rather than what they can't. We find that it's been helpful to document for when students transition from primary to secondary, or when they relocate to another country / school. Still lots of room for growth though!
